Submit a referral
We accept referrals from insurers, employers, legal counsel, and other authorized parties. Choose your service line below or follow the general steps on this page.
How to refer
- 1Prepare a referral letter with file details, evaluee information, and specific questions for the assessor.
- 2Gather relevant clinical records, prior reports, and supporting documentation.
- 3Email the referral package to assessments@fdguelph.ca or call 519-763-4406 to discuss intake.
- 4Our team confirms receipt, assigns an assessor, and coordinates scheduling with the evaluee.
What to include
- Referral letter with clear questions and contact details
- Relevant clinical records and treatment summaries
- Prior IME or expert reports where applicable
- Agreed or requested report deadline if applicable
