Employer & Disability Referrals
Refer fitness-for-duty, FAE, or disability assessments with job demands and return-to-work context where available.
Steps to refer
- 1Submit a referral with claim details and assessment objectives.
- 2Include job description or demands analysis if applicable.
- 3Email documents to assessments@fdguelph.ca or contact intake by phone.
- 4We schedule the assessment and confirm report delivery expectations.
Include in your referral package
- Employer or insurer claim reference
- Job title, duties, and physical or cognitive demands
- Current work status and accommodation history
- Clinical records and prior functional assessments
- Return-to-work goals and timeline constraints
