Overview

Frontier Medical Assessments, a division of Frontier Diagnostics, collects and uses personal health information to conduct independent medical assessments, coordinate scheduling, and deliver reports to authorized referrers. We handle information in accordance with applicable privacy legislation, including the Personal Health Information Protection Act, 2004 (PHIPA) where applicable.

Information we collect

We may collect identifying information, clinical records provided by referrers, assessment findings, correspondence related to scheduling, and billing or administrative details necessary to complete the assessment process.

How we use information

Information is used to schedule and conduct assessments, prepare independent reports for referring parties, communicate with authorized stakeholders, and meet legal and regulatory obligations. We do not sell personal health information.
