Steps to refer
- 1Prepare a referral outlining the policy or claim context and assessment questions.
- 2Include clinical records, prior insurer medical reviews, and functional documentation.
- 3Send the package to assessments@fdguelph.ca.
- 4We confirm intake, match a specialist, and schedule the assessment.

- Policy or claim reference and coverage type
- Diagnosis and treatment history
- Current functional status and restrictions
- Prior insurer medical opinions
