Steps to refer
- 1Prepare a referral letter outlining the file, claim number, and specific questions.
- 2Gather relevant clinical records, prior IME reports, and imaging summaries.
- 3Email the referral package to assessments@fdguelph.ca or call 519-763-4406.
- 4Our team confirms receipt, assigns an assessor, and coordinates scheduling with the evaluee.
Include in your referral package
- Claim number and insurer contact details
- Date of loss and mechanism of injury
- Specific questions for the assessor
- Prior treatment summaries and IME history
- Any SABS forms or treatment plans under review
